Adelaide M. Haywood, known as Addie, was born in October 1865 in Nebraska, a daughter of Charles Haywood; her father had been born in England and her mother in Illinois.1–2 The family tree and WikiTree give her birth as 3 October 1865 at Elk Creek, in Johnson County.3
On 2 April 1885, at Auburn, Nemaha County, nineteen-year-old Addie M. Haywood married James Harrison Cook, a young Nebraskan from Otoe County.2, 4 They settled at Julian, in northern Nemaha County, where James farmed and then went into business; in June 1900 he was a lumberman, and the couple owned their house. Addie had borne seven children by then, six of them living, and six were at home: Arthur, Lena E., Hazel, Blanche, Charles H. and Calixte C.1 By 1919 she had had nine children in all, of whom one son had died.4
About 1911 the Cooks moved to Spokane, Washington, where James dealt in real estate. When he died there of tuberculosis on 1 November 1919, "Ada M." gave the details for his death certificate, and with his mother she took his body home to Nebraska for the funeral at Julian and burial in the Camp Creek cemetery.4–5 Their daughter Hazel was then Mrs. Arthur Curtis of Spencer, Iowa, and Blanche was Mrs. Shiffer of Spokane.4, 6–7
Adelaide M. Cook died in November 1922, three years a widow, and was buried in the Camp Creek Cemetery in Otoe County, where her husband lay.4, 8 Her daughters remembered her as Adelaide: Blanche gave her mother's name as Adelaide M. Heywood to the Social Security Administration, and when Hazel died in 1978 her death certificate gave her mother's maiden name as Adelaide Haywood.9–10 Hazel had named her own daughter Adelaide in 1920, and it was that daughter, Adelaide Hegnauer, who gave the particulars for Hazel's certificate.10

Haywood in her own line of her 1885 marriage record, which also names her father, Charles Haywood;2 Addie M. in the 1900 census;1 Addie M. Heywood in her husband's obituary;4 Ada M. Cook on his death certificate;5 Adelaide M. Cook on Find a Grave;8 Adelaide M. Heywood in her daughter Blanche's Social Security claim;9 and Adelaide Haywood as the mother's maiden name on her daughter Hazel's death certificate.10 Addie is the usual pet form of Adelaide, and Ada another short form; two of her daughters, independently, gave Adelaide, and Hazel named a daughter Adelaide.
