Lewis O. Curtis was born in August 1884, probably in Illinois, the third child and second son of Nathan K. Curtis and Louisa Thomas.1–2 His parents had married about 1872 and were living in Porter County, Indiana, with his sister Viola in 1880, the year his brother Arthur Garfield was born there; some time after that they moved west into Illinois.

In June 1900 Lewis, fifteen, was living with his parents on a rented farm in Ontario Township, Knox County, in western Illinois, with his younger sisters Jennie and Edna and a hired boarder. Arthur, eighteen, was boarding and working on another farm in the same township.1 By 1910 his parents had moved on to Spencer, Iowa, with only Edna still at home.3 What became of Lewis after 1900 has not yet been found.
